Grand Prize: (1 winner) Family Vacation to Universal Orlando Resort consisting of round-trip coach class airfare from major airport nearest to the GrandPrize Winner’s residence to Orlando, FL; roundtrip ground transportation between airport and hotel; 4-days/3-nights at the Universal Orlando Resort (one standard room; quad occupancy); and up to four 4-day All Park Passes to Universal Studios Florida. If the Grand Prize Winner resides within 250 miles of Orlando, FL, s/he will receive ground transportation for up to four persons to the Universal Orlando Resort instead of airfare (ARV: $4,620). If the actual retail value is less than its stated Approximate Retail Value, the Grand Prize Winner will not receive the difference in value. All expenses not specified above, such as meals, beverages, mini-bar, entertainment, telephones, souvenirs, gratuities, personal or spa services, additional guests, etc., are solely the Grand Prize Winner’s responsibility. All accommodations and reservations will be as arranged by Co-Sponsors and are subject to 30 day advance notice, holiday black-out dates and availability. Trip must be completed by May 31, 2012. Other Trip restrictions may apply. Winner and guests must possess required travel documents and must travel together on same itinerary. Co-Sponsors have no responsibility for lost, stolen, misplaced or mutilated tickets and/or travel documents, baggage, flight delays, or any other occurrences beyond its control. Each Grand Prize Winner's guest must be 21 years of age or older unless the Grand Prize Winner is the guest’s parent, legal guardian or spouse. The potential Grand Prize Winner will be notified by email/phone or mail on or about May 2, 2011. The potential Grand Prize Winner, or the potential Grand Prize Winner’s parent/legal guardian if the potential Grand Prize Winner is a minor in his/her state of residence, will be required to sign and return a notarized Affidavit of Eligibility/Compliance and Publicity/Liability Release, where allowed by law, and such other documents as may be reasonably requested by Co-Sponsors. Failure to sign and return these documents by the return date specified in the prize notification will result in forfeiture of the entire Grand Prize, and, time allowing, the selection of an alternate potential Grand Prize Winner. The Grand Prize Winner will be issued a Form 1099 for tax purposes in the amount of the actual retail value of the Grand Prize and must submit his/her Social Security Number, as required by law. Odds of winning the Grand Prize Sweepstakes depend upon the total number of all eligible entries received during the Promotion Period. NOTE: Regardless of Prize won, it is not redeemable for cash; non-transferable or substitutable except by Co-Sponsors, who reserve the right, in their sole discretion, to substitute a Prize or portion of a Prize with a prize of equal or greater value. All costs and expenses not specifically included above, including all federal, state and local taxes, are solely the responsibility of the Weekly Sweepstakes or Grand Prize Winner (collectively, referred to as the “Winners”).
